PARTICIPATION AGREEMENT

Gateways of the Mind, Claire E Morency and Claire Morency Hypnotherapy and her agents provides the following services:

Hypnotherapy, Self-hypnosis Training, NLP, Conscious Language, Clarity Coaching and Body Language Translations and a host of other technologies and methods. And, provides this information as the United States of America 1st Amendment in action.

Hypnotherapy, Words, Body language and Clarity Coaching are an educational process facilitating access to internal resources which assist men and women in solving problems, increasing motivation, and altering behavior patterns to create positive changes.

Education and training in hypnotherapy are classified under Human Services in the Health and Human Services Division of the classification of Instructional Programs by the United States Department of Education.

Her Office is in Kittitas county, Washington and she serves the world:  Virtually and on Zoom

This information will be used to aid in serving you as the client.  Please write clearly, answer honestly and know, answering yes or no to any question does not mean you cannot receive services from this practitioner.  Your honest answer serves in your receipt of appropriate care and service.  All information will be kept confidential within the health insurance Portability and Accountability Acts (HIPAAA) regulations.

Limits on Confidentiality of Information

Clients have a right to expect information revealed in session is not disclosed without extraordinary justification.  The conditions that justify the release of information and by law must be reported to the appropriate agencies are the following:

            •           Knowledge of child abuse or neglect.

            •           Knowledge of senior citizen abuse or neglect.

            •           A client poses a serious risk of suicide and is an imminent danger to self.

            •           A client poses a threat of imminent danger to another person.

            •           By a court issued subpoena, may obtain information.

            •           Report to law enforcement authorities, knowledge of a felony that has been or is being committed.

In other situations, signed authorization for release of information is required.

Release of Liability Agreement:

I personally know of no case on record where an individual has been harmed using these methods.  I do know of thousands of cases where men and women of all walks of life have benefited greatly from the use of these methods.  As a general practice, it is necessary for everyone taking part in private sessions, classes, workshops, and seminars with Claire Morency Hypnotherapy, to sign this Release of Liability Agreement. 

Hypnotherapy and coaching practices are not the practice of Medicine or psychotherapy, nor are they offered as a replacement for them.  Hypnotherapy is an Adjunctive Modality to other Medical or psychiatric treatments. If you choose to act on ideas found here, you do so by your personal choices. Self-healing and therapies of this type requires intelligence, informed decisions and your ability to take responsibility for your own actions. 

Payments: All payments are due prior to services rendered.  Cancellation or Rescheduling requires a 24 hours advanced notice. Day-of session, non-emergency cancellations are charged a $90 cancellation fee. Payments for hypnotherapy, coaching, workshops  or event services must be used within a 90-day period.

Agreement:

I am of legal age, and in consideration of my acceptance as a participant in this private hypnotherapy and coaching session, seminar, or workshop, I, for myself, my heirs, my executors, administrators, and assignees, do hereby release and discharge Claire E Morency, Claire Morency Hypnotherapy, Gateway to Fulfillment from all claims of damages arising from, or growing out of my participation in said activities.  I agree and hold myself responsible for my physical, mental and spiritual health care. I am fully responsible for myself. I use any ideas found here by consulting my inner guidance, a medical professional or I am a researcher or health care provider myself.

•   I am an active participant in my experience and see myself as a partner in the transformative nature of this process.

•    I recognize my thoughts, feelings, images, and actions have a direct effect on circumstances and direction of my life.

•     My well-being is directly related to how I care for myself physically, emotionally, intellectually, and spiritually.

•     I take responsibility for my experience of life.

•     I agree to be on time for my sessions and allow a minimum of 24 hours advance notice if I need to cancel or reschedule a session or I will be charged a $90 cancellation fee.

Claire Morency’s First session fee is $220 and will be 2 to 3 hours in length. Each session thereafter is offered with package discounts and are from 60 to 90 minutes in length.
